Horizontal Preparation Protocol (HPP)

by Dr. Andi Dragus

HPP is a functional, minimum invasive protocol, which is predictible and can be easily reproduced. The aim of the HPP is to realize the required preparations for veneers and crowns.

HPP is comprised of 5 components of clinical and phyisical analysis.

This polishing protocol has been invented by Dr. Andi Dragus in 2016 and presented in Germany at Komet who took it over and produces currently 3 different sets of burrs to utilise the HPP. One set for veneers, one for crowns and one for both veneers and crowns.

  • The HPP concept starts from the horizontal plan of the dental roots.
  • The horizontal plan of the dental roots present an irregular and unique contour of each dental root.
  • On this horizontal plan the dentist has to realise the marginal closure of the facade or crown.
  • It is important to localize mentally in 3D depth the dental root section that is to be polished with threshold.

By looking at this horizontal plan of the detnal root before starting the preparation the dentist obtains valuable information about the dental root contour, incline in oral-vestibular plan and mezio-distal and the ratio between this HP and the dental roots of the neighbour teeth that directly influence the burr incline needed during the mechanical preparation, as well as the future contact surface of the veneers or the crowns.
When controlling this horizontal plan which serves the threshold of preparation the 1st medical objective represented by the marginal closure, which is directly influenced by the clasic or digital print, is accomplished.
It is very important that the marginal closure is realised with a single material and not with two.
The vertical plan of the preparation is influenced also by the HP of the dental roots and the vertical non-retention is realised mechanically using the length of the burr, while the top of the burr is supported by the HP during the preparation.

The HP is encountered on the dental roots, at the periodontal level, at the occlusion plan, at the TMJ (temporomandibular junction) plan in three of the mandibular dynamic parameters ISS, SA, BENETT ANGLE.
When achieving the balance between all the HP of the stomatognathic system we can provide the patient with a prosthetic work that is functional and esthetic, without any medium or long term associated risks for the craniomandibular system (CMS).

The HPP is integrated in the concept designed by Dr Andi DragusCondilographiated Veneers which uses the condilography diagnosis as a basis to identify the mandibular dynamic and the masticatory reflex, with the help of the Arcus Digma condilograph.
After the condilography, all the data obtained for the functional and esthetic treatment plan will be sent to the technical laboratory together with the position of the skull determined with the facial bow.

All veneers and ceramic or monolithic crowns are realised extra oral in a dental technics laboratory.
